Comment Card Image Files

• OMB Control #0710-0019, expiration has been extended to 02/28/2023 without any change of previously approved collection. We have made an update to images to reflect the new expiration date.
  OMB has granted an extension while new survey is moving through the approval process.

  IMPORTANT:

  1. NEW THIS YEAR: NO SEPARATE SURVEY PROCESS FOR VISITOR CENTERS. Visitor centers are no longer considered as stand-alone PSAs; thus, there is no more need to conduct a separate survey process for visitor centers. Visitor centers are now included in the list of recreation areas in the Survey Calendar Generator.
  2. Please DISCARD all old cardsand do NOT use leftover cards from previous years.

  3. Please download the new image files for your 2022-23 comment card survey and follow the Printing Instructions properly.

  4. The PDF version of the survey has added features allowing project to select the survey protocol, and type in the date, project name and recreation area/visitor center name. On the AGENCY DISCLOSURE STATEMENT page, Responses should be directed to: Please Update with your project name and address.
