    DD Form 1805 (Violation Notice) - Additional Ordering Information
    1. A number of districts have been able to order a supply of the DD Form 1805 from the Army Media Distribution Division (MDD) in St. Louis. Others have not been as successful.

    2. Please remember that these forms must be ordered by each district (not project) "through normal ordering channels". This means the specific district office that has placed previous orders for the DD 1805 and has established an account with the MDD is the only office authorized to place an order. Depending on the district, the authorized office could be Information Management, Publications Officer, Logistics, Operations, etc.

    3. I have been working with both the MDD and the Department of the Army - and collectively, we have been able to identify the account numbers for each district. This account number must be referenced when placing a district order. For security reasons, I can not (nor can MDD) provide the account numbers. The district person/office authorized to place an order should contact Mary Pople, Inventory Management Specialist, Logistics Management Branch, MDD, at 314-592-0900 ext 8420 to begin the ordering process or to obtain additional information.

    4. The MDD has now been instructed to provide one violation book per project. When placing your combined district order, please order only this amount. Remember additional quantities will be available within a month.
