New DD 1805 Form (Violation Notice) Available On 14 December 2005
1. I have just received word from the Office of the Provost Marshal General, that the new DD Form 1805 (Violation Notice) will be available this Wednesday, 14 December, from the Army Media Distribution Division (MDD). The new forms should be obtained through normal procurement channels from the MDD, Directorate of Logistics, St. Louis, Missouri.

2.Please note that only 1750 books will be available from the first printing, but that an additional 12,000 books will be printed and available in mid-January. Because of the limited initial amount, the MDD will limit the number of books ordered per location.

3. Many of you may have already received the attached memorandum from the Central Violations Bureau. The memo provides a reminder to destroy the old DD Form 1805 upon receipt of the new form. It also advises that violations issued on the old form may be subject to dismissal by the court.

4. Due to the limited number of forms, I highly recommend you place your order as soon as possible.
