Approved OF-301 Volunteer Forms
Please see the attached OMB approved OF-301 volunteer forms (OF-301 Volunteer Application; OF-301a Volunteer Agreement; OF-301b Volunteer Group Sign-up) and instruction document for completing the forms. These forms replace the OF-301 and OF- 301a that expired in 2014. In addition, a new group form: OF-301b has been approved for use in conjunction with the OF 301a.

Volunteers currently signed up on the expired OF-301a do not need to sign a new agreement. However, if any conditions change (contact information, volunteer duties, reimbursements, etc), the volunteer will need to complete and sign the new version of the OF-301a form.

These common forms have been approved for use by the Department of Agriculture (Forest Service, National Resources Conservation Service); Department of the Interior (National Park Service, Fish and Wildlife Service, Bureau of Land Management, Bureau of Reclamation, U.S. Geological Survey); Department of Defense (U.S. Army Corps of Engineers); and Department of Commerce (National Oceanic and Atmospheric Administration).

These forms are approved for use through 12/31/2017, at which point an updated version will be submitted to OMB for approval. All forms and instructions will be posted later this evening on the NRM Gateway Volunteer Forms page. Any FAQs that are developed as the new forms are implemented will be added to the Volunteer FAQ page.
