• Printing Instructions (Please review and follow the instructions before printing the image cards)

  IMPORTANT:

  1. Please DISCARD all old cards and do NOT use leftover cards from previous years for both Recreation Area and Visitor Center.

  2. Please download the new image files for your 2018-19 comment card survey and follow the Printing Instructions properly.

  3. The PDF version of the survey has added features allowing project to select the survey protocol, and type in the date, project name and recreation area/visitor center name. On the AGENCY DISCLOSURE STATEMENT page, Responses should be directed to: Please Update with your project name and address.
