Ordering a Supply of Cards

    A new OMB-approved visitor comment card (OMB 0710-001, Exp. 31 March 2006) became available for use in April 2005.

    Projects that plan to conduct comment card surveys during the FY05-06 recreation season for submission to RecBEST will be asked to administer between 120 and 480 completed comment cards for each project or lake that is surveyed. The number of cards needed to conduct surveys at any given project will be determined during the survey planning exercise. Typically, surveys for each project or lake will be conducted with one of the following sample size options:

    Ace Printing and Blueprint Co. will accept orders by telephone and payment by credit card. Note that the cost quotes provided below do not include the UPS shipping charge. The shipping charge amount will be calculated when you order. Because of the substantial cost savings, we encourage Division or Project Offices to make a bulk purchase and distribute cards to their projects.

    Two important notes:

    • Do not use the existing comment card for surveys conducted this year. The data entry program in RecBEST will only accept data from the new card.
    • The Office of Management and Budget (OMB) approved this survey on 01 April 2005. The approval applies to all Projects that plan to conduct this survey. There is no need to submit a separate application for OMB clearance.
